Physical Examination

A physical exam must be provided prior to the start of classes in the fall and cannot expire before the end of the academic year. Students must have their physician complete a Physical Health Report form.

Please note, the physical examination must include a Tuberculin Skin Test (PPD). 

• If this is your first PPD, you are required to complete a two-step exam. One exam with a negative reading and a retest in 1-3 weeks
• If you have had a PPD within the year, please provide a copy along with your updated PPD.

Any positive PPD/allergic reaction requires a negative chest x-ray. A chest x-ray is required every two years or Quantiferon Gold blood test yearly.
