Current Students - Immunization Requirements (Health Records)
Immunization: New York Public Health Law, Article 21, requires that all college students born after January 1, 1957 and enrolled in six (6) or more credits in a semester, demonstrate proof of immunity against measles, mumps and rubella. Proof of immunity consists of:
- Measles: two doses of live measles vaccine administered on or after the student's first birthday, physician documentation of measles disease or blood titer showing immunity;
- Mumps: one dose of live mumps vaccine administered on or after the student's first birthday, physician documentation of mumps disease or blood titer showing immunity;
- Rubella: one dose of live rubella vaccine administered on or after the student's first birthday or blood titer showing immunity. Physician's diagnosis is not acceptable.
Meningococcal Information Acknowledgement: New York Public Health Law, Article 21, requires that all college students that are enrolled in six (6) or more credits in a semester receive and acknowledge information from the college regarding meningococcal disease and vaccination, whether they live on or off campus.

Proof of immunity must be submitted within thirty (30) days from the start of classes. Students should check with their high school health office or health care provider to insure they meet these requirements. Failure to submit proof of immunization can result in prohibition from classes, exclusion from registration or withholding of grades, transcripts and future registration. If prohibited from courses, students may receive an "F" grade and will not receive refunds or fees after the College's normal refund period.
In addition to these requirements for every student, there are additional health requirements forLPN's, RN's, Radiological Technology, Massage Therapy, Wilderness Recreation, and Student Athletes.
Additional Health Requirements for LPN's, RN's and Radiological Technology Students:
- Physical: must be a recent physical (within 1 year) and be updated annually. (*NCCC does not recommend that you have a physical dated more than 90 days prior to the start of of the semester.)
- TB Skin Test (PPD): must be recent (within 1 year) and be updated annually.
- Varicella / Zoster and Hepatitis B Information Sheets: forms must signed and returned annually.
- Confidentiality Statement must be read, signed and returned annually.
- Current CPR Certification: must obtain CPR Certification for Adult, Child, Infant, AED and updated as needed (NCCC offers CPR training monthly).
- *RN Students must also submit a copy of their current New York State LPN License prior to the start of classes.

- Physical: must be a recent physical (within 1 year) and be updated annually. (*NCCC does not recommend that you have a physical dated more than 90 days prior to the start of of the semester.)
- Physician must also fill out information sheet for Wilderness Recreation Students included with the Physical Form.
- Release Form: must be read, signed and returned annually.
- Confidentiality Statement must be read, signed and returned annually.
Additional Health Requirements for Student Athletes:

Where can I obtain my immunization records from?
Contact the high school that you last attended. They may have the necessary information that can be forwarded to our office. If you are an armed service veteran, you may have received the necessary immunizations through the military and can forward a copy of the records to the Records Office for review. Your family doctor may also have this information and can make a copy or fill out the purple student immunization record form. |

What if I have a medical reason why I should not receive the MMR vaccinations?
Your doctor must certify in writing the specific reasons why you should not receive the vaccination and an approximate date on which you will be able to receive the vaccination. |

What if I can not afford to receive the vaccination?
You should contact your local County Public Health Office. They may offer the MMR vaccination to college students for $3.00-$5.00 and Meningitis vaccinations for $65.00-$95.00. |

What are the penalties for failure to comply?
You will be prohibited from attending classes if all information is not received in the Records Office within 30 days from the start of classes. A hold will be placed on your account not allowing you to register for future classes at NCCC nor will your grades or transcripts be issued until you have complied with New York State Public Health Law. |
